NIH DEPARTMENT OF PERIOPERATIVE MEDICINE PRIVACY NOTICE

Privacy Policy: This service is provided and hosted by Recreational Solutions (RecSolutions), a division of Cartagena, Fontenot & Matheny Enterprises, Inc. The site will collect first name, last name, middle initial, telephone number, e-mail, address data including a primary and secondary address, city, state, zip, as well as a registration group code. This information will be provided to the National Institutes of Health (NIH), NIH Department of Perioperative Medicine. The information collected reflects the minimum necessary to accomplish the purpose of class enrollment.

Purposes and Uses

The data is collected for the purpose of enrolling students in courses offered by NIH. This data will be stored on our servers for a period of six years. We do not use your personally identifiable information for any other purpose except as explained in this Notice. RecSolutions does not display any ads nor send any emails to anyone that signs up for a class. Your email address and other personal information will never be sold or shared with any third parties without your consent.

By signing up for a class, you agree to have your information shared with the Coordinators, if any, for your class. Your name and contact information may be printed on rosters and attendance sheets as necessary for running the class you sign up for. You agree to receive emails from the administrators and Coordinators as they deem necessary for administration of your class. You may choose to opt in to receive optional notifications such as reminders the day before any class sessions you are signed up for. Coordinators and other authorized NIH users (as applicable) must use a username and password to retrieve this data.

Please note that by entering data into this site, you are accepting the practices described in this Privacy Notice and consent to the collection, storage, and use of personal information about you as described in this Notice. Use of this service is optional. If you do not wish to use this service, but still wish to enroll in a class, you may call (301) 402-0059 to enroll over the phone. Please be aware that even in that instance, your information will still be entered onto this site by an administrative user at NIH and stored on our servers. Failure to supply the information requested will prevent enrollment in a class.

Security

RecSolutions maintains physical, electronic and procedural safeguards to comply with federal standards to guard your nonpublic personal information. The system uses appropriate administrative, technical and physical controls for protection of information contained therein and is protected by multiple firewalls, vulnerability scanning, and advanced encryption technology. Access to student information requires entry of a personalized username and password. Remote access to the RecSolutions servers is not permitted. RecSolutions does not believe that other privacy risks exist; if this should change, RecSolutions will disclose those risks as well as a plan to mitigate those risks. In non-routine circumstances, system administrators at RecSolutions may have incidental exposure to your information during unexpected events such as data recovery.

RecSolutions does not make use of persistent cookies, "web bugs," or other tracking technologies. We use cookies that are session only i.e. they last for only one single web browsing session. We also use standard Google Analytics that collects anonymous traffic activity on the website. The information collected is anonymous to the effect that it can’t uniquely identify any user or their related activity. Web server logs are maintained and include time of visit and IP address. This information is used solely for webserver performance metrics and is not used for tracking, nor is the information shared. Our e-mails may contain small electronic files known as web beacons (also referred to as clear gifs, pixel tags and single-pixel gifs). Web beacons differ from cookies in that the information is not stored on your hard drive, but invisibly embedded on web pages or in email. Web beacons permit us to track online movements of web users -- for example, to count users who have opened an e-mail.

Authority

The legal authority to solicit and collect the course enrollment information sought by this system is 42 USC 241, 263, and 282.

Though the data entered will not be stored on NIH servers, the data will be visible to NIH authorized staff who may use all or part of it to create and print class rosters for their reference. The NIH Privacy Policy applies and is located at http://www.nih.gov/about/privacy.htm.
